Patient Referral, Retention and Management

Medicaid enrollment is rising rapidly, with total enrollment projected to reach 78.5 million in 2022. Powered by the Innovaccer Health Cloud, our Managed Medicaid solution was launched earlier this year to help risk-based providers maximize contract performance, employ a collaborative care model, ensure easy access to community-based resources, and improve quality and social determinants of health (SDoH) outcomes. The solution significantly improves the health of Medicaid populations by integrating our Unified Data Model with powerful analytics and boosts physicians' efficiency with point-of-care alerts on care gaps, coding gaps, medication adherence, and more. On the other hand, since patients can now select their preferred providers under the new MA plans, providers are looking to ensure that patients stay in their network with better care quality and patient engagement. They also need strong network analytics to track performance, identify high-performing physicians, and lead informed conversations.

Innovaccer launched its Referral Management solution to help healthcare organizations manage all referrals in one place to minimize network leakage and improve health outcomes. It allows providers to initiate referrals at the point of care with the right insights. It also creates a two-way communication channel with patients and automates processes to close feedback loops.

Pediatric Care

Today’s pediatric care organizations need innovative healthcare solutions to effectively manage care delivery for young patients and identify health risks earlier, Innovaccer launched its Pediatric Care Management solution earlier this year to unify pediatric care information from multiple venues, clinicians, and stages of a child’s life. The solution focuses on delivering family-centered care and effectively managing children’s health across the care continuum.

Organizations can seamlessly update patient records to create unique EMPIs and ensure that every newborn has unique IDs. With its robust, AI-enabled patient-matching algorithms, the solution allows providers to flag any potential redundancies in patient records with more than 99% accuracy. It also uses more than 40 HEDIS-certified pediatric measures to help pediatric healthcare organizations keep a close eye on the health of every patient and triage them based on their utilization and risk scores.

The 6th Annual Population Health Payer Plan Innovations for Medicaid, Medicare & Duals took place virtually in May. This conference is the only event designed exclusively for payers and delivers cutting-edge tools and strategies to operationalize SDoH for new VBCmodels. More than 20 top health plan leaders shared best practices and case studies focused on data, analytics, and technology; integrating SDoH, behavioral health, and health equity; and building partnerships with communities and providers.

NAACOS held its spring conference virtually in April, featuring leading ACO experts and CMS officials. Experts from Innovaccer discussed current ACO challenges and presented a report on the views of 3,000 VBC leaders.

At the 2021 Population Health Management Forum For Payers & Providers, industry leaders connected to identify best practices and strategies to address the challenges of improving and maintaining a healthy population while reducing healthcare spending.

HIMSS 2021 recently gathered a variety of attendees from across the health IT ecosystem—CIOs, senior executives, providers, IT professionals, government officials, innovators, consultants, market suppliers, and more—to accelerate healthcare innovation, offer educational programs, networking events, and exhibitions from August 9 to 13, 2021, in Las Vegas.

Innovaccer will attend the Virtual Collocated Value-Based Payment Summits, which brings together ACO/Direct Contracting Summit XI, Bundled Payment Summit X, Medical Home/Advanced Primary Care Summit IX, MACRA MIPS/APM Summit V, and Medicare Advantage Summit III. It will be hosted as a virtual event from September 20 to 23, 2021.

Now more than ever, Medicaid leaders need to more effectively and rapidly address a growing at-risk population’s physical and behavioral needs, engage with members, and improve outcomes. Those attending the 2021 Medicaid Managed Care Summit (October 4–5, 2021, at Loews Miami Beach Hotel in Florida) will learn what leaders are doing to respond to the changes resulting from the pandemic.

As Senior Director of Health Outcomes, Dr. Shobha Phansalkar will lead Innovaccer’s clinical research to identify the health outcomes associated with Innovaccer’s solutions. An internationally recognized expert in clinical decision support, Dr. Phansalkar has published more than 50 peer-reviewed articles and book chapters. She has led research for the National Institutes of Health, the Agency for Healthcare Research and Quality, the U.S. Department of Veterans Affairs, and the Office of the National Coordinator for Health Information Technology.

As Senior Director of Value Engineering, Daniel William Roberts will implement best practices to enhance the VBC offering on the Innovaccer Health Cloud. He’ll help customers drive value in their operations, bringing more than 15 years of clinical leadership experience and deep knowledge of complex healthcare systems, contract negotiations, and aligning patient outcomes with financial models critical to VBC.
