KLAS Recognizes Innovaccer's Data Activation Platform among the technologies to succeed in Value-based care

The report is focused on organizations that use their population health solution specifically for care management and Social Determinants of Health (SDOH). The report calls out Innovaccer for ‘strong data aggregation performance and nimble development.’ 

Value-based contracts and patient populations vary, so do the technology needs of provider organizations. As participation in value-based contracts has exploded, so have healthcare organizations’ efforts to take on population health management (PHM). This has prompted many discussions about the best ways to identify at-risk patients, engage them in their health, and track the outcomes of those efforts.

Care managers are typically tasked with the largest portion of this work. Leveraging registries, claims data and other patient-data repositories, they seek to identify care gaps, create patient care plans and document progress. Today, most care managers are not utilizing vendor software to perform many aspects of this work instead, they still rely on manual workflows. A small but increasing number of organizations are turning to their PHM vendors to automate their workflows so that they can pursue care management work more efficiently. 

Innovaccer was featured in the Evaluation Performance Report for Population Health Management in 2018 and evaluated again in 2019. In 2019, Innovaccer was assessed on value-based care technologies in population health management, care management and the social determinants of health. Innovaccer has emerged with a KLAS rating of 89.9* while the average of the category stands at 81.7. Innovaccer received an above-average score of 92.1 for its relationships with customer healthcare organizations. Delivery of new technology also emerged as a strong forte for Innovaccer with a score 1.4 points higher than the segment average of 7.1. 

True care management requires collaboration among stakeholders in multiple areas.

Many technology offerings have gaps and the adoption of available functionality is often very early. EMR vendors often offer many capabilities and have solid adoption, but customer satisfaction tends to be lower. 

Innovaccer was recognized as offering all three major capabilities required to drive successful care management; identifying and closing gaps, offering longitudinal records, and delivering actionable insights from complex data. According to the report, “Users can generate a clear view of patient history thanks to strong data reconciliation and interfaces with clinical and claims systems. Innovaccer continues to develop a longitudinal record that can be customized to an organization’s focus or goals.” With Azure Cloud, they are further finetuning these capabilities. 

Our care-management platform is within the Innovaccer product. Everything that is documented by our care management staff is tracked across our network and has a really nice timeline. We can scroll through the timeline and get a good story of what is going on with the patient.

SDOH capabilities are driven primarily by customer needs and organizations, especially more advanced ones, are pushing vendors to pull in an assessment of and clinical data on SDOH.

KLAS also asked participating provider organizations about their ability to leverage data on SDOH which is notoriously tricky to gather, manage and make accessible through their PHM system. However, the adoption of tools to obtain SDoH data is very low today.

Innovaccer offers a standard SDOH assessment and the system is able to pull information into this assessment from different sources such as the EMR or other tools patients may use to enter relevant data. Customers report that Innovaccer is flexible in meeting customers’ SDOH needs and typically builds the functionality to meet their specific needs. Once the SDOH data is delivered, it is typically searchable and customers can create drill-down reporting to retrieve patient-level insights. Beyond the SDOH assessment, Innovaccer also offers tools that draw on data-regression techniques to provide population-level insights based on zip codes. Community-based social resources and their data has also been linked to the platform to address care and SDOH needs.

The Innovaccer system very much so provides actionable insights around social determinants of health. We picked the tool because it was on the cutting edge in terms of the use of social determinants of health. The tool has a social vulnerability index, which is like a risk assessment that we can use with our case managers. We can do a thorough assessment in real-time, and that helps us target the social determinants of health. The tool puts those things forth as primary information, and that is something we find to be important.

KLAS researchers believe that there is an opportunity for vendors to facilitate and encourage more interfaces and closer connections with referral organizations and community resources—leading to improved patient care. Provider organizations also have a responsibility to bridge relationship gaps between their organization and community-based organizations to better incorporate community resource data to address SDOH for patients.
