About the Presenters

Paul Grundy MD, MPH, FACOEM, FACPM
Founder of PCPCC, Chief Transformation Officer at Innovaccer & “Godfather” of the PCMH movement

Dr. Grundy is the Chief Transformation Officer at Innovaccer. Affectionately known as the ‘Godfather’ of the Patient-Centered Medical Home model, he is the Founding President of the Primary Care Collaborative (PCC) and a member of the National Academy of Medicine. Prior to Innovaccer Dr Grundy enjoyed an esteemed career at IBM, as the Chief Medical Officer of the Healthcare Life Sciences division, and the Global Director of Healthcare Transformation. Previous to IBM he was the Minister Consular for the US Department of State. Dr. Grundy is a globally respected healthcare convener, storyteller, and visionary - a "Trusted Healer”. He holds numerous international honorary titles and awards, including an honorary lifetime member of the UK National Association of Primary Care, an Ambassador for Healthcare DENMARK, and the first international member of the Irish National Association of General Practice. He was the recipient of the NCQA National Quality Award in 2012, and recipient of the prestigious Barbara Starfield Primary Care Leadership Award in 2016.

Fran Soistman is the Founder and President of Healthcare Management and Transformation Advisory Services LLC (HMTAS). Prior to this he was Executive Vice President at CVS Health-Aetna , where he led their Government Services Business portfolio, the growth engine of the company. Fran also represented Aetna on the Health Care Transformation Task Force, and served as its Chairman until his retirement from CVS Health- Aetna on October 1, 2019. He was a recipient of the 2004 Ernst & Young Entrepreneur of the Year award in the Healthcare Technology/Services category. From 2004 to 2008, he was appointed by Pennsylvania Governor Rendell to serve on the Health Care Cost Containment Council, providing direction for the state agency charged with addressing growing health care costs.
