Dr. Grundy is the Chief Transformation Officer at Innovaccer. Affectionately known as the ‘Godfather’ of the Patient-Centered Medical Home model, he is the Founding President of the Primary Care Collaborative (PCC) and a member of the National Academy of Medicine. Prior to Innovaccer Dr Grundy enjoyed an esteemed career at IBM, as the Chief Medical Officer of the Healthcare Life Sciences division, and the Global Director of Healthcare Transformation. Previous to IBM he was the Minister Consular for the US Department of State. Dr. Grundy is a globally respected healthcare convener, storyteller, and visionary - a "Trusted Healer”. He holds numerous international honorary titles and awards, including an honorary lifetime member of the UK National Association of Primary Care, an Ambassador for Healthcare DENMARK, and the first international member of the Irish National Association of General Practice. He was the recipient of the NCQA National Quality Award in 2012, and recipient of the prestigious Barbara Starfield Primary Care Leadership Award in 2016.

An entrepreneurial executive with technology-savvy and leadership experience across multiple start-ups and fast-growing companies, Evan retains deep familiarity with ASP/SaaS business models.

Ambulatory Care Program Manager, MercyOne Population Health Service Organization (PHSO)

Emily Fletcher is the Ambulatory Care Program Manager at MercyOne Population Health Service Organization (PHSO). She leads the integration of Health Coaches, Social Workers, Pharmacists, and Community Health Workers into the medical home to positively impact consumer experience and patient outcomes. Emily received her Bachelor’s degree in Socio-Political Communications from Missouri State University and a Master’s degree in Public Health from the University of North Texas Health Science Center. She completed her practicum with the Centers for Medicare and Medicaid Services in Dallas, Texas. Emily started at MercyOne in 2010, working in oncology research for five years before joining the PHSO in 2016. She served a two-year term as Vice Chair of the Institutional Review Board. Emily’s goal is to create a health care environment where all patients have the same opportunity to be healthy. Addressing health-related social needs through the community health worker program is a key strategy in accomplishing this goal.

Rakshay Jain is the senior director of product management at Innovaccer. He has over 10+ years of experience in healthcare management and has helped payors, providers, and pharma companies on a variety of strategic issues. Prior to joining Innovaccer, he was an Associate Director at Navigant where he was leading delivery and business development of projects to providers, primarily focusing on the following issues: enterprise strategy, value-based care strategy, facilities strategy, financial planning, mergers & acquisitions, and digital health. He also holds a Masters Degree in Business Administration from Northwestern University– Kellogg School of Management, and a Bachelor of Technology from the Indian Institute of Technology (IIT) Delhi.
