About the Virtual Session

Patients, healthcare organizations, government and social agencies, and payers are suddenly collaborating with urgency in response to the ongoing Covid-19 pandemic. The Healthcare industry has a new mindset; we’ve hit the tipping point, and there is no going back to yesterday’s way of doing things. Teams across the healthcare continuum are reimagining themselves in the framework of this new normal. How will we keep clinicians and patients engaged as the population health management model evolves? How do our definitions of risk change? And what role do virtual care and an activated data platform play in this new way of making sense of the world?

Join this exclusive webinar with David B. Nash, MD, MBA, Founding Dean Emeritus, Jefferson College of Population Health and Jason Krupp, MD, President, Central Maine Medical Group to hear how we can achieve new outcomes in the new normal of the healthcare industry.

You will learn

  • How has “normal” shifted for population health risk specifically and for the industry stakeholders in general? How will top performers drive healthcare value in the new normal?
  • How do we engage both patients AND clinicians against this backdrop?
  • How do we fast track the measurement and management of Social Determinants of Health (SDoH), and move this into the population health management workflow?
  • What role does virtual care - telemedicine and beyond - play in measuring and managing risk? How does it impact our outcomes? Our predictive models?
  • What is the most essential data for top performance in Value-Based Care?
