WHAT IS CMS ACCESS?

A Medicare program built around continuous chronic care.

The ACCESS Model tests an outcome-aligned payment approach in Original Medicare to expand access to new technology-supported care options that help people improve their health and prevent and manage chronic disease. The voluntary model focuses on conditions affecting more than two-thirds of people with Medicare, including high blood pressure, diabetes, chronic musculoskeletal pain, and depression.
Your doctor referred you because they believe you would benefit from more regular support. Trinity Health has partnered with Story Health to deliver that support, and your care team stays involved the entire time.

Your existing Medicare benefits stay exactly the same. Nothing changes about your coverage, providers, or services.

Your coach checks in regularly, answers your questions, and keeps you on track between appointments.
Monitoring from home
We send you a connected device - like a blood pressure cuff— at no cost. Your readings go directly to your care team.
Clinical oversight, when you need it
A Story Health nurse practitioner reviews your care plan and can adjust your treatment without a clinic visit.
Your doctor in the loop
We share structured updates with Trinity Health throughout your program. They see every change to your care plan.

What to expect after you enroll.

Meet your health coach
Within a few days of enrolling, your health coach reaches out to introduce themselves, review your care plan, and get you set up — including any monitoring devices you need.

An initial clinical visit
If appropriate, A story health clinician will reach out to schedule a virtual visit to establish your clinical baseline, review your medications and activate your care plan. The visit summary is shared with your care team

Ongoing support between visits
Your health coach checks in regularly, supports medication adherence, and flags concerns to a clinician when something needs attention. If your condition changes, we coordinate directly with your doctor.

Regular updates to your doctor
We send structured clinical updates to your doctor at Trinity Health throughout your program — your clinical measures, medications, and any changes to your care plan.
How we work

Who this program is for.

You may be eligible if you have Original Medicare (not Medicare Advantage) and one or more of these conditions:
High blood pressure
Hypertension
High cholesterol
Dyslipidemia
Prediabetes or obesity
Early Cardio-Kidney Metabolic conditions
Diabetes or heart disease
Advanced CKM, CKD Stage 3, or ASCVD
Your care team at Trinity Health can confirm whether you qualify and help you enroll.

Common Questions

Do I have to enroll?
No. This program is completely voluntary. Your doctor referred you because they believe it would help, but the decision is yours.
Will I still see my regular doctor?
Yes. Your doctor stays involved throughout. All your regular appointments, specialist visits, labs, and imaging remain with Trinity Health. Story Health supports your care between those visits.
Does this cost anything?
Nothing. CMS ACCESS is a Medicare benefit. There is no copay and no out-of-pocket cost for enrolling in this program. You will never receive a bill from Story Health by Innovaccer.
What information is shared about me?
We send regular updates to your doctor at Trinity Health — your clinical measures, medications, and care plan progress. All sharing requires your consent at enrollment and complies with HIPAA and other relevant legal and regulatory requirements.
Can I leave the program?
Yes, at any time. Contact your health coach or call the number on your enrollment materials.
How long does the program last?
The initial program period is 12 months. At the end, you and your care team will review your progress and discuss whether to continue.
Do I have access to a human health coach 24/7?
You have access to support whenever you need it. During business hours, your health coach is a message away. Around the clock, AI monitors your readings and flags anything that needs attention. You are never on your own.
