“The solution allows us to effectively drive workflows. For example, we are utilizing the tool to determine the right level of resources that we need to assign to a patient. For example, if a patient has a high social need, we actually assign a community health worker or a social worker to that patient. If a patient has a high medical need, then we assign an RN. The solution takes some of the thinking and guesswork out of the process, but we are really able to drive our own internal workflow so that we can be as efficient and effective as possible.”
