A leading clinically integrated network (CIN) in Central Florida with more than 275,000 patients and 5,000 physicians needed to reduce its readmission rate of 16.3% in 2019. Since its inception in 2013, the CIN has grown by more than 300% and has served around 481,000 unique members. The network observed that 37% of its patients were readmitted at least once, staying for an average of 6.5 days. Repeat admission diagnoses accounted for 12% of all readmissions. To reduce readmission rates, the network leveraged the Innovaccer Health Cloud to implement well-designed, automated protocols and manage care transitions. Using the Innovaccer Health Cloud, providers could build personalized care plans and close gaps in care.
