As part of the healthcare industry’s growing shift toward holistic patient care, the role of social determinants of health (SDoH)—the conditions in which people are born, grow, live, work, and age—has also increased.
Innovaccer’s InNote solution, which delivers point-of-care technology for smarter, holistic patient care, now offers the community benefit organizations (CBO) referral tool.
Using the CBO referral in InNote, providers are able to identify any SDoH factors as part of a patients’ office visit and refer them to relevant community resources – food, housing, and financial and legal help, in addition to standard health services – based on those needs.
1. Display the identified risks
Based on a patient’s latest assessment of the social determinants of health, providers can display any identified risks.
2. View referral status
Users get a single-platform view of all completed community referrals, as well as the status of any existing or ongoing referrals.
3. Filter and assign resources
This feature allows the user to search through a list of available community resources, where the provider can then refer community resources to the patient.
4. Share details
The product users can share all pertinent details of the relevant community resource(s) with the patient via third-party platforms.
